What happens if you find something worse?

Work stops and you get a revised quote before anything continues. That is the rule regardless of how far into a job we are. If we open up an area expecting a loose fastener and find a damaged joist or water-affected subfloor, that is a different scope and it should be your decision whether and how to proceed, not a surprise on the invoice.
